Pay: $42-$48/hr, DOE

Key Responsibilities:

• Coordinate and manage the MDS/RAI process
• Ensure accurate and timely completion of assessments
• Collaborate with interdisciplinary team for care planning
• Monitor quality indicators and assist with regulatory compliance
• Serve as a resource for clinical documentation improvement

What We're Looking For:

• Active California LVN or RN license
• Prior experience in MDS coordination and PPS preferred
• Knowledge of PDPM and care planning
• Strong communication, assessment, and analytical skills
• Team player with a positive, solution-focused attitude
