Hospice Certified Nursing Assistants (Healthcare)

Traditions Home Health Services is currently seeking compassionate, skilled, reliable, well-polished and experienced Hospice trained Certified Nursing Assistants to assist with our client who is currently on Hospice who resides in the Milton area.

Shifts available:
Days: Monday through Sunday 8am-8pm (12-hour shifts)
OVN: Monday through Sunday 8pm-8am (12-hour shifts)

CNA Job Responsibilities:
- Assisting with daily living activities, including toileting, incontinence care, bathing, dressing, grooming, nail care, oral hygiene, and skin integrity maintenance. CNA would document care and findings.
- Preparing healthy, fresh meals in accordance with the client's care plan, if required and ensuring adequate fluid intake throughout the shift and documenting client's intake.
- Adhering to infection control protocols.
- Monitor clients skin integrity, doing skin checks daily and documenting findings.
- Administering medication reminders and assisting with the application of eye drops, ointments, creams, and hospice medications
- Continuously monitor/assist client for all transfers and ambulation as client is a fall risk.
- Skilled in assisting with transfers, operating mechanical lifts, if required
- Proficient in dementia care.
- Proficient in hospice care
- Competently documenting shift activities digitally hourly using a smartphone
- Certified Nursing Assistant would report any concerns to the Traditions Home Health Services Office Support Team in a timely manner
- Certified Nursing Assistant would work with the Hospice agency and treat staff with respect and as colleagues.
