Nursing Home Case Manager OBRA (Healthcare)

LifeWays is looking for a Nursing Home Case Manager to join our team of compassionate, driven professionals. In this role, youll provide trauma-informed, culturally sensitive mental health support to individuals in nursing facilities and carry a small case load of community-based consumers. Youll help people navigate systems, access services, and build a life of dignity, independence, and hope.

What Youll Do
Complete OBRA psychosocial assessments and ongoing mental health monitoringDevelop and implement Individual Plans of Service (IPOS) using person-centered planningCoordinate care and connect individuals with financial, medical, and community supportsSupport transitionswhether from hospital to home, nursing facility to community, or school to workProvide crisis intervention and post-hospitalization planningCollaborate with a variety of providers to ensure holistic, consistent careAdvocate fiercely for the people you serve
What Were Looking For
A degree in social workExperience with case management and/or mental health services (preferred)Strong organizational and communication skillsA passion for helping others live their best lives
