The Lingenfelter Center (TLC) was established in 2006 when it became clear that Mohave County was in need of a care facility for those suffering from Alzheimer's/Dementia. Our dedication to our residents and our ongoing training and support for our staff allowed for TLC to be named one of America's top 100 nursing homes by U.S. News and World Report in 2010, 2013, 2014, 2015, 2016 & 2018. TLC also has an A+ rating with the BBB.

We're looking for a dynamic and caring individual to join our skilled nursing facility as our Director of Social Services--someone who's ready to lead with empathy, coordinate with confidence, and bring comfort and support to the residents who call our facility home.
In this role, you'll be the heart of our social services program--connecting residents and their families with the resources they need, helping make smooth transitions into our care, and ensuring our residents are supported emotionally, mentally, and socially throughout their stay.
Essential Job Functions
· Be a champion for our residents, advocating for their physical, mental, and emotional well-being
· Lead the charge on initial assessments, personalized care plans, and family support meetings
· Coordinate smooth, compassionate admissions and ensure every resident's paperwork and records are in perfect order and follow admission guidelines.
· Stay plugged into our community resources, so families always know where to turn
· Play a key role in census development by conducting tours and helping prospective residents feel at home
· Be a go-to team member for resident progress updates, discharge planning, and everything in between
· Keep us compliant and confident by maintaining accurate documentation and adhering to all applicable regulations


Knowledge, Skills and Abilities
· Experience in social work preferably working directly with patients and their families in a skilled nursing facility.
· Organized, independent, and not afraid of a busy, ever-changing day
· Excel at connecting with people, even in tough or emotional situations
· Great interpersonal communication skills
· Attention to detail with record keeping
· Have strong computer skills (EMRs, Microsoft Office, emails etc.)
· Are bilingual? That's a huge plus!
· Friendly, polite and courteous with high levels of patience, empathy and compassion
Required Education and Experience
· 1+ year of social work experience in a healthcare setting
· Ability to lift/move up to 30 lbs occasionally (yes, sometimes you'll carry more than emotional weight!)
· CPR certified, AZ Fingerprint Clearance Card, and up-to-date on required health screenings
· Comfortable working around residents who may be physically or emotionally challenging
· Must be able to sit, talk, listen, and engage for long periods
Schedule
This is a full-time position at a 24/7 facility, primarily Monday-Friday during the day, but flexibility is key--some evenings, holidays, or weekends may pop up from time to time.
