DIRECTOR OF CARE COORDINATION RN - JOB SUMMARY:

Here at BridgePoint, the Director of Care Coordination - Registered Nurse is responsible for leading efforts to design and develop all care management services for BridgePoint Healthcare District of Columbia locations and Jefferson Parish in Louisiana. The RN Director of Care Coordination will inform and execute the strategic direction and brand of care management for BridgePoint Healthcare that aligns with long term vision and goals.  The RN Director of Care Coordination will be responsible to develop, implement and monitor all day-to-day operations and employees for care coordination services.

Director Care Coordination, Dir Care Coord, RN Director, Nursing Director Care Coordination, Nurse Director, Case Management Director, RN Case Management Manager

Education: BSN required and Masters in health-related field preferred.

Licenses/Certification: Current Registered Nurse RN license in the District of Columbia.

Experience: Minimum of three (3) years case management experience to include leadership and management. Minimum of five (5) years acute care nursing experience
