Investigator / Incident Resource Manager (Finance)

As the Investigator / Incident Resource Manager, you will ensure the health, safety, and protection of individuals with intellectual and developmental disabilities (I/DD) receiving services. You will conducti timely, objective, and thorough investigations into reportable incidents and ensure compliance with all applicable regulations, provide technical assistance and coaching to program staff, and collaborate with regulatory agencies to promote a culture of accountability, transparency, and safety.

Key Responsibilities

  • Conduct comprehensive investigations of reportable incidents, including interviews, fact-finding, and documentation.
  • Collaborate with internal a nd external stakeholders including OPWDD and the NYS Justice Center.
  • Prepare clear, timely, and objective reports in compliance with OPWDD Part 624/625 and Justice Center requirements.
  • Support root cause analysis and recommend corrective and preventive action plans.
  • Monitor incident trends, compile data reports, and identify opportunities for systemic improvements.
  • Provide training and technical assistance to staff on incident reporting and response protocols.
  • Ensure timely submission and accurate entry of incident information into the Incident Management System (IRMA).
  • Maintain confidentiality and high professional standards at all times.
  • Participate in agency committees (e.g., Incident Review, Human Rights, Informed Consent) as assigned.
  • Conduct follow-up and post-investigation monitoring to ensure implementation of recommendations.

Qualifications

Qualifications

  • Bachelor's degree in Human Services, Criminal Justice, Psychology, Social Work, or a related field (Master's preferred).
  • Minimum of 3 years of experience conducting investigations or managing incidents in a human services setting.
  • Strong understanding of OPWDD regulations, NYS Justice Center standards, and incident management best practices.
  • Excellent interviewing, analytical, writing, and communication skills.
  • Ability to manage multiple priorities and meet deadlines.
  • Experience with IRMA, electronic health records, and investigation tracking tools preferred.
  • Flexibility to travel to sites and participate in on-call coverage as needed.

Work Environment

This position is hybrid with a minimum of 3 on-site days per week in a primarily office-based environment. On-site days may vary depending on department needs and supervisor's discretion. Regular travel to program locations across the five boroughs is expected. The role may involve remote documentation, on-call responsibilities, and in-person presence during critical incident reviews.

Position Type/Expected Hours of Work

This is a full-time position, typically Monday through Friday, 9 a.m. to 5 p.m. Flexibility is required to address incidents or participate in reviews outside of core business hours.
